SSCTC provides the following services to inmates living with HIV who are preparing to be released from anywhere in the state to the South Shore area:
- Short-term, intensive case management and reintegration services
- Development and implementation of individualized service plans
- Transportation - upon release and for medical and related appointments
- Personal care products
- Emergency food vouchers
- Assistance with access to
- Health insurance
- Medical and mental health care
- Substance abuse treatment
- Housing or residential treatment facilities
