Our Framework and Techniques are scalable to your needs, timeline and budget. We work well with a team approach or independent worker.

To assist organizations, coalitions and communities that work with vulnerable populations to eliminate health disparities and create healthy communities to achieve their mission and measure their outcomes.


 

Service Framework

The Outcomes Project developed the following framework after years of working in a collaborative manner with our community partners. Our Service Framework makes it easy to organize a project into achievable phases with specific outcomes.

  • Visioning Phase is a process that develops a high-level concept of what the desired end-state of the project will look like after it has been implemented.
  • Assessment Phase is a process to analyze the need or issue in a community or program.
  • Capacity Building Phase is a process that defines and obtains the resources needed to implement the project.
  • Implementation Phase encompasses all the activities needed to reach the project’s goals.
  • Evaluation Phase analyzes the results of the project and determines what worked well and what can be improved upon before implementing a similar project in the future.


Service Techniques

Our Service Techniques are tools that that produce high-quality deliverables and measurable results.

  • Meeting Facilitation provides neutral facilitators to assist organizations in using their internal expertise to reach goals and objectives.
  • Survey Administration assists organizations with administering surveys and evaluating their results.
  • Education and Training delivers courses that help organizations develop specialized skills as well as develop customized training to meet our clients’ specific needs.
  • Grant Coaching provides coaches to assist organizations in developing their grant proposals.
  • Focus Group Facilitation conducts focus groups in order to gain more in-depth information from your target audience. 
  • Key Informant Interview conducts key informant interviews to collect qualitative information at a personal level.
