The Plymouth AIDS Support Services (P.A.S.S.) provides services to people living with HIV as well as provides support and educational information to their partners, families and the community.

Programs

P.A.S.S. consists of Client Services and The South Shore Corrections to Community Reintegration (SSCTC). These programs are funded by Boston Public Health Commission through the Ryan White Treatment Modernization Act Title 1, the Massachusetts Department of Public Health and private donations.

Client Services provides the following services to those living with HIV:

  • Case Management/client advocacy
  • Food vouchers; nutritional supplements
  • Assistance with transportation, housing, MassHealth and legal issues
  • Referrals to medical, dental and mental health care services; substance abuse treatment, HIV testing and social services
  • Free condoms, dental dams, lubricants and bleach kits

The South Shore Corrections to Community Reintegration Program (SSCTC)

SSCTC provides the following services to inmates living with HIV who are preparing to be released from anywhere in the state to the South Shore area:

  • Short-term, intensive case management and reintegration services
  • Development and implementation of individualized service plans
  • Transportation - upon release and for medical and related appointments
  • Personal care products
  • Emergency food vouchers
  • Assistance with access to
    • Health insurance
    • Medical and mental health care
    • Substance abuse treatment
    • Housing or residential treatment facilities

Partners

P.A.S.S. collaborates with several area AIDS service organizations to provide comprehensive HIV/AIDS care. Father Bill's Place and Adult Family Care are just two such service programs. Adult Family Care is located with P.A.S.S. at 123-1 Camelot Drive in Plymouth.

The Plymouth area office provides the following services:

  • Housing search and homelessness prevention
  • Assistance with applications for housing subsidies
  • Applications for assisted living and congregate housing programs
  • Access to rental start-up and homelessness prevention funds for those eligible
  • Home visits for those unable to travel
  • Advocacy of all housing issues - including CORI and credit problems
  • Stabilization services after housing is achieved
  • Information and referrals to local services

Adult Family Care

Family Service Association of Greater Fall River

The Adult Family Care Program provides foster care for any adult in Massachusetts on standard MassHealth with a medical or psychosocial need.

S/he must require assistance with his/her healthcare management and minimal basic activities of daily living in order to remain in the community.

This program also serves adults living with HIV/AIDS, Hepatitis C, substance abuse issues or those who are developmentally disabled.

A nurse/social worker team provides services and support for the Greater Plymouth area.
